【摘 要】As populations age, all adult-onset diseases become more common. Importantly too, more people have more than one illness and many have multiple illnesses. Patients who have many interacting medical and social problems are frail. When they become acutely ill, they challenge modern medical services which, for the most part, have been developed to care for people with illness in a single system.
